If the patient is deemed eligible for medical cannabis following the consultation, complete the patient’s Medical Document. Like a prescription, the Medical Document must be signed by a healthcare professional. Healthcare professionals should mail or send the original Medical Document to the Vitalicann Care Team via secure fax at 1-844-762-1802.
